Online Booking Terms and Conditions

These Terms and Conditions (“Terms”) apply to all online bookings made with Life Mental Health (“the Practice”, “we”, “our”, or “us”). By making a booking through our online system, you (“the client”, “you”, or “your”) agree to be bound by these Terms.

1. Purpose of Online Booking

1.1. The online booking system is provided for your convenience to schedule appointments with our qualified mental health practitioners.
1.2. All bookings are subject to practitioner availability and confirmation.

2. Client Information

2.1. You agree to provide accurate, current, and complete information when making an online booking.
2.2. You are responsible for updating your contact details so we can reach you regarding your appointment.
2.3. If booking on behalf of another person (e.g., a child or dependent), you warrant that you have the authority to do so.

3. Appointment Confirmation

3.1. You will receive an email and/or SMS confirmation of your booking.
3.2. The Practice reserves the right to reschedule or cancel an appointment in the event of practitioner illness, emergency, or unforeseen circumstances. In such cases, we will use reasonable efforts to notify you as soon as possible and assist in rescheduling.

4. Fees and Payment

4.1. Consultation fees are as published on our website, available from reception, or otherwise communicated at the time of booking.
4.2. Fees may be payable at the time of booking, or on the day of the appointment, depending on the practitioner’s policy.
4.3. Medicare rebates, private health insurance, or third-party funding (e.g., NDIS, WorkCover, EAP) may apply where eligible. It is your responsibility to confirm your eligibility.

5. Cancellations, Rescheduling, and Non-Attendance

5.1. We require at least 24 hours’ notice if you wish to cancel or reschedule an appointment.
5.2. Cancellations made with less than 24 hours’ notice, or non-attendance, may incur a cancellation fee of up to 100% of the consultation fee.
5.3. Repeated cancellations or non-attendance may result in restrictions on future bookings.
5.4. If you are late to an appointment, the session may still end at the scheduled time, and the full fee may apply.

6. Use of Telehealth Services

6.1. Some appointments may be offered via telehealth (telephone or video consultation).
6.2. By booking a telehealth appointment, you consent to receiving services through this medium and agree to ensure a private, safe, and stable environment for the session.
6.3. It is your responsibility to ensure adequate technology and internet connection. Technical difficulties on your end may still result in fees being charged.

7. Privacy and Confidentiality

7.1. The Practice complies with the Privacy Act 1988 (Cth) and the Australian Privacy Principles, as well as relevant Queensland health records legislation.
7.2. Your personal and health information will only be collected, stored, used, and disclosed in accordance with our Privacy Policy, available upon request.
7.3. Information shared with your practitioner is confidential, except where disclosure is required or authorised by law (e.g., risk of harm, child protection, court orders).

8. Duty of Care and Emergency Situations

8.1. Online booking is not suitable for crisis or emergency situations.
8.2. If you are in immediate distress or at risk of harm, please call 000 (emergency services) or contact Lifeline on 13 11 14.
8.3. Appointments made through our system are not guaranteed to be immediately available; waiting times may apply.

9. Limitation of Liability

9.1. While we take all reasonable care in providing our online booking service, we do not guarantee it will be error-free or uninterrupted.
9.2. To the maximum extent permitted by law, the Practice is not liable for any loss, damage, or injury suffered as a result of using the online booking system or attending (or not attending) an appointment.

10. Amendments

10.1. We may update these Terms from time to time. Updated Terms will be posted on our website and apply immediately to all future bookings.

11. Governing Law

11.1. These Terms are governed by the laws of Queensland, Australia.
11.2. Any disputes shall be subject to the exclusive jurisdiction of the courts of Queensland.

By proceeding with an online booking, you confirm that you have read, understood, and agree to these Terms and Conditions.
