Cancellations made within 24 hours of your scheduled appointment will incur the full session fee.
Late payments may result in a temporary suspension of services until the outstanding balance is settled.

Updated information

Please inform your therapist of any changes or updates to your physical health at the beginning of each session to ensure safe and appropriate therapeutic support.

Risk of physical activity

Dance Movement Therapy involves physical activity that may include stretching, balance work, expressive movement, and somatic exploration. While every effort is made to tailor sessions to individual capabilities and ensure safety, participation carries inherent risks.

By engaging in this service, you acknowledge that:

You are physically able to participate in movement-based therapy, or have consulted a medical professional regarding any limitations.
You understand the potential risks associated with physical activity, including but not limited to muscle strain, fatigue, or injury.

Confidentiality

In order to provide you with the service and/or treatment plan appropriate to your care, it is necessary for The Base Health to collect and record personal information that is relevant to your situation. Your personal information is gathered as part of the assessment and treatment and is retained in order to document clinical sessions and to enable your clinician(s) to provide relevant and informed mental health services to you. This information is stored in our cloud-based practice management software program, Halaxy.
Our clinic is bound by the legal requirement of the Australian Privacy Principles (APPs) contained in the Privacy Act 1988 and our own Privacy Policy (available on request).
All personal information gathered by The Base Health during your child’s treatment will remain confidential and secure, except where:
1. Failure to disclose the information would place you, or another person at serious and imminent risk; or
2. There is an obligation to disclose information under the Children and Young Persons (Care and Protection) Act 1998 (NSW); or
3. There is an obligation to disclose information due to a breach of ethical conduct by a registered Health Professional; or
4. It is subpoenaed by court; or
5. It is used in relation to defending The Base health in legal proceedings or for obtaining advice in respect of any potential legal proceedings; or
6. De-identified information is discussed as part of an approved professional supervision process; or
7. Your prior approval has been obtained to:
a) Provide a written report to another professional or agency (e.g., GP, Specialist); or
b) Discuss the material with another person (e.g., a family member, Specialist, case manager etc); or
8. If disclosure is otherwise required or authorised by law.
