This is required to obtain consent and facilitate Medicare rebate processing. The form takes approximately 5 minutes to read and complete. To avoid delays, please complete the Online Intake Form as soon as possible after booking. ________________________________________ Fees and Rebates Standard appointments are approximately 50 minutes, with the remaining time in the hour used for clinical documentation and therapy planning. Standard Fees • Mitch Hart: $280 • Genja Lusnats: $270 • Jason Leong: $260 (Saturday appointments $275; Couples $280) • Dr Henry Austin: $250 (Child & Adolescent and Couples appointments $270) Medicare Rebates A Medicare rebate of approximately $149 per standard consultation may be available for up to 10 sessions per calendar year when referred by a GP or Psychiatrist under a Mental Health Care Plan. If your policy includes eligible extras cover, Private Health Fund rebates may also be available for appointments with a Clinical Psychologist.
