Terms & Conditions – Exercise Physiology Consultations

1. Booking Policy

By booking an appointment online, you agree to the following terms and conditions. All appointments are subject to availability and confirmation by the clinic.

2. Appointment Duration & Scheduling

When booking online, you will be asked to select a preferred time block (morning, midday, or afternoon). This is a request only, and your exact appointment time will be confirmed by our team once availability is reviewed.

Standard consultations run for the allocated duration as advised upon confirmation. Please arrive on time to ensure you receive your full session. Late arrival may result in a shortened appointment out of consideration for other clients.

3. Cancellation & Rescheduling Policy

We require a minimum of 24 hours’ notice to cancel or reschedule an appointment.

Cancellations or reschedules made less than 24 hours before the appointment may incur a cancellation fee of up to 100% of the session cost.

Non-attendance (“no-shows”) will be charged the full session fee.

Exceptions may be made for emergencies or unforeseen circumstances at the discretion of the clinic.

4. Fees & Payment

Payment is required at the time of consultation unless otherwise arranged (e.g., via WorkCover, DVA, NDIS, Medicare, or third-party referral).

Out-of-pocket fees are due on the day of service.

Rebates may apply depending on your referral type and eligibility.

5. Health & Safety

Please inform your Exercise Physiologist of any new injuries, illnesses, or health changes prior to your session.
Your program will be tailored to your current condition, and it is your responsibility to follow instructions and exercise safely.
The clinic is not liable for injuries sustained as a result of failure to disclose relevant health information or disregard of professional guidance.

6. Privacy & Confidentiality

All personal and health information is managed in accordance with the Privacy Act 1988 (Cth) and Australian Privacy Principles (APPs).
Your details are stored securely within the Halaxy platform and are only shared with other health professionals with your consent or when required by law.

7. Referrals & Reporting

If your sessions are part of a referral program (e.g., DVA, Medicare, NDIS, WorkCover), relevant reports will be provided to your referring practitioner as required for ongoing care and funding compliance.

9. Agreement

By booking and attending your appointment, you acknowledge that:

You have read and understood these terms and conditions.

You consent to receive Exercise Physiology services as outlined.

You accept responsibility for providing accurate health information and following professional advice during sessions.
