Informed Consent to Assessment and Treatment

I consent to receive assessment and treatment from Myotherapist and Acupuncturist Luke Troiani.

I understand that treatment may include myotherapy techniques, soft tissue therapy, trigger point therapy, dry needling, acupuncture, cupping, joint mobilisation, stretching, exercise rehabilitation, corrective exercise, movement assessment, and related therapeutic or educational interventions.

I understand that treatment will be based on clinical reasoning and my presenting condition, and that the nature, purpose, and expected benefits of treatment will be explained to me as required.

I acknowledge that while treatment is generally considered safe, there are potential risks and side effects, including temporary soreness, bruising, fatigue, dizziness, mild swelling, temporary symptom aggravation, skin irritation, and in rare cases infection, nerve irritation, or complications associated with needling techniques such as pneumothorax. I understand that no guarantees can be made regarding treatment outcomes.

Where applicable, I consent to the use of acupuncture and/or dry needling, understanding that sterile single-use needles will be used and that minor bleeding, bruising, or post-treatment soreness may occur. I understand I may decline or withdraw consent for needling at any time.

I confirm that I have provided accurate and complete information regarding my medical history, medications, injuries, surgeries, and relevant health conditions, and agree to inform my practitioner of any changes to my health status during the course of treatment.

I understand that my personal and health information will be collected and stored in accordance with the Privacy Act 1988 (Cth), and will remain confidential except where disclosure is required by law, necessary to prevent serious risk to health or safety, or authorised by my consent.

I acknowledge that all services are provided in accordance with Australian Consumer Law, including that services will be delivered with due care and skill. I understand that individual responses to treatment vary and that no guarantees have been made regarding outcomes, recovery times, or results.

I understand that treatment is voluntary and I may refuse or discontinue any aspect of care at any time.

By proceeding with treatment, I confirm that I have read and understood this consent form and voluntarily agree to assessment and treatment.
