OT Inspire Australia- Client Agreement & Consent (Please note hours will still need to be provided in your Service Agreement)

This agreement is provided for the purpose of providing support under the National Disability Insurance Scheme in line with the participant’s NDIS plan. This Agreement is made according to the rules and the goals of the National Disability Insurance Scheme (NDIS).

The participant and the service provider agree that this Agreement is in line with the main ideas of the NDIS which includes:

support the independence and social and economic participation of people with disability
enable people with disability to exercise choice and control in the pursuit of their goals and the planning and delivery of their supports.

These ideas include things like having more choices, achieving your goals and taking part in the community.

OT Inspire is committed to provide services in a holistic, individual and family centred way. This includes detailed intervention planning and goal setting to ensure all parties involved are working towards the same objective.

All therapeutic services (inclusive of assessments) are charged at the clinical rate per hour in accordance with the NDIS price guide and participant locations.

Cancellation Policy/ Terms and Conditions:

OT Inspire request 48 hours notice for appointment cancellations. If a participant makes a short-notice cancellation, within 48 hours before the service, OT Inspire charge up to 90% of the price for the cancelled appointment (this includes travel costs in remote and very remote locations).
Preparation time and record keeping will be considered in the hours of service accounted for in your plan. Each appointment with your therapist will include 15 minutes of intervention planning as a standard fee. Any additional services such as communication with other stakeholders or resource production (e.g. visuals) will incur additional costs.
Travel is billed at the clinical rate. Remote and Very Remote travel costs will be shared between participants in the same region in remote or very remote areas where possible.
All urban services (MM1-MM5) (including but not limited to assessments, liaison, communications on behalf of client, travel, reports, AT requests) are billed at the clinical rate for remote locations of $193.99 per hour. Therapy Assistants are billed at $86.79 per hour.
All remote (MM6) services (including but not limited to assessments, liaison, communications on behalf of client, travel, reports, AT requests) are billed at the clinical rate for remote locations of $271.59 per hour. Therapy Assistants are billed at $121.51 per hour
All very remote (MM7) services (including but not limited to assessments, liaison, communications on behalf of client, travel, reports, AT requests) are billed at the clinical rate for very remote locations of $290.99 per hour.

OT Inspire work towards providing a high quality of person-centred goals and therapeutic intervention. Client Rights & Responsibilities

I agree to provide the required sections of my NDIS plan to ensure services can be provided as funded by the NDIS (e.g. NDIS goals/ plan manager details) where applicable at least 48 hours prior to my first appointment.
I am aware that intervention sessions alone may not be sufficient to demonstrate progress. Therefore, I agree to actively participate in the - intervention sessions and understand that OT Inspire may recommend additional exercises outside of intervention sessions.
I will advise OT Inspire of any changes to my personal details (contact number, address) or details of my NDIS plan (additional allied health professionals involvement, increase in funds, plan amendments etc.).
I understand that it is my responsibility to inform OT Inspire as soon as possible 48 Hours minimum if I or my child is unable to attend intervention sessions. Some unforeseen circumstances may allow for a cancellation at a later time. I accept that failure to notify OT Inspire may incur a fee of 90% of the session.
I acknowledge that repeated cancellations or failure to attend without notification may result in cessation of intervention services for myself or my child. The decision is at the discretion of OT Inspire.
I am aware that I can cease intervention services at any time. I commit to advise my therapist if I intend to leave the service and that already provided services will be invoiced. I understand that if OT Inspire cease services for any reason I will be notified as soon as possible.
I understand that failure of payment for services, may result in reports being withheld or sessions being placed on hold until payment has been made.
I understand that it is my responsibility to notify OT Inspire should my funding decrease or change.
I understand that there are some instances in which my therapist may have to break confidentiality in order to keep me safe.
I understand if I have any concerns I can also discuss this with my treating therapist or manager at OT Inspire.

OT Inspire Responsibilities:

OT Inspire is committed to provide services in a holistic, individual and family centred way. This includes detailed intervention planning and goal setting to ensure all parties involved are working towards the same objective.
OT Inspire agree to treat all information provided to them as private and confidential and will ensure safe and secure storage of any written documentation.
If OT Inspire is unable to provide services due to illness or any reason you will be advised as soon as possible.
