We liaise with medical teams and supports to ensure a holistic care approach.
We provide care plans for clients with complex needs.
We provide nursing and continence reports including consumables quotes for NDIS plan reviews.
We provide services to all funding streams, NDIS, ICWA and private. • Comprehensive understanding of discharge planning complexities and community access.
