Terms and Conditions for Room hire:

A $25 deposit is required at least two weeks prior to to confirm and hold your booking.

Please email proof of payment to admin@formotionphysio.com.au once this is done. Note if payment is not received your booking may be cancelled. The deposit paid will be applied to your final invoice.
One week's notice is required for any cancellations or changes. Less notice will result in us retaining your deposit.
Professional Indemnity and Public Liability insurance is required for all customers, please email to admin@formotionphysio.com.au to complete your booking. Please note door access will not be given until this is done.
Access is for room and furniture use only, all consumables (ie creams / towels / supplies) need to be supplied by you and taken home each evening unless specific permission is requested to store overnight.
Rooms must be left tidy on leaving.
