Informed Consent, Privacy and Service Agreement

Privacy and Confidentiality
Your privacy is very important. All information collected by your treating practitioner (psychologist, mental health occupational therapist, or social worker) is kept confidential and used only to provide you with an effective and appropriate service. Your information is stored securely in accordance with relevant legislation and professional standards.

Clinical records are maintained to document your treatment, support continuity of care, and meet legal requirements. Records are retained for a minimum of 7 years after your last contact, or—if you were under 18 at the time of service—until you reach 25 years of age.

You may request access to your information at any time. Your practitioner will discuss the appropriate and safe way to do this in line with legal, ethical, and clinical requirements.

Limits to Confidentiality
Information you provide will remain confidential except in the following circumstances, where disclosure may be required:

Risk of harm: If there is serious or imminent danger to your safety or the safety of another person.
Legal obligations: If information is subpoenaed by a court or required by law.
Professional consultation: When practitioners seek peer consultation or supervision to provide you with the best possible care. Only information necessary for the consultation is shared, and all consultants are bound by confidentiality.
Your consent: If you provide written or verbal permission for information to be shared with another party (e.g., GP, psychiatrist, lawyer, teacher, employer, family member).
Government-funded programs: If your referral is part of a government-funded scheme (e.g., Mental Health Treatment Plan), it is standard practice to provide brief progress reports back to the referrer. You may withdraw consent, though in some programs this may limit your eligibility to continue services.

Fees and Cancellations

Medicare consultation (50 minutes) - Psychologist: $170.90
Medicare consultation (50 minutes) - Clinical Psychologist: $220
Medicare consultation (50 Minutes) - MHOT, MHSW: $159.20
Couples or family session fee: $210
DVA, NDIS, VOC, WorkCover, CTP - Sessions charged to the relevant scheme at the relevant standard rate

If you have a valid referral (e.g., Mental Health Treatment Plan), you may be eligible for a Medicare rebate for part of the session fee.

Cancellations:

You may cancel or reschedule your appointment at no cost if you provide at least 2 business days’ notice.
Cancellations made with less than 2 business days’ notice, or non-attendance, incur the full session fee, as your appointment time is reserved exclusively for you.
Please note that Medicare, DVA, WorkCover and CTP schemes do not cover cancellation fees. In these cases, cancellation charges are billed directly to clients.
Dauntless offers the option of a phone or telehealth appointment in place of a cancellation. Choosing this option avoids any cancellation charges.

By attending your appointment, you acknowledge that:

You have read and understood the information above.
You have had the opportunity to ask questions.
You consent to receive psychological, mental-health occupational therapy, or social work services from Dauntless.
You understand the limits of confidentiality and your rights regarding your information.
