1. Mental Health Treatment Plan - In Clinic Appointment (Please ensure you provide your GP referral before your first session)

This appointment is for clients who have a current Mental Health Treatment Plan and GP referral and wish to access Medicare rebates for their sessions. Before Your Appointment: - Please ensure you have a valid Mental Health Treatment Plan and referral from your GP. - You will either need to upload a copy of the plan and referral with your intake forms, email it to hello@mindfulmen.com.au or bring it to your first session. - Medicare rebates can only be processed once we have received your paperwork. Fees & Rebates: - A 50% booking fee is required at the time of booking to secure your appointment. - The full session fee is payable on the day of your appointment. - A Medicare rebate will be processed back to your nominated Medicare account.
