Please upload a copy of the participant's NDIS plan and any other relevant documentation (e.g., diagnostic letters, reports, assessments, etc.)
Once this form is completed, I will be in touch to arrange an initial assessment over phone call. This call usually takes around 45 minutes to 1 hour (sometimes shorter) and is billable under NDIS. I always do my best to keep the call efficient while gathering the information needed for therapy planning.
