CONSENT FORM

‘Exhale Exercise Physiology’ is an Exercise Science/Exercise Physiology based business, offering face-to-face and online consultations. Exhale Exercise Physiology’s mission is empower individuals to feel strong in their body and peaceful in their minds.

PERSONAL INFORMATION:
I understand that I am required to provide personal details, including my medical history in order to allow Exhale Exercise Physiology to properly assess and provide exercise prescription to ensure my safety and health care needs.
I grant permission for Exhale Exercise Physiology to contact me via SMS, telephone, email, and social media messaging where necessary for appointments and billing purposes.
I provide permission for Exhale Exercise Physiology to be in contact with other health professionals, for the purpose of creating a multidisciplinary team approach for my care, and to discuss my treatment and management approaches.
I understand I am able to discuss my care at Exhale Exercise Physiology to other associated health professionals at any time.

I understand that a Telehealth consultation is an online video consultation. By connecting to a Telehealth video consultations, I am consenting to the consultation.

PRIVACY:
Exhale Exercise Physiology respects your rights to privacy and takes confidentiality very seriously. Exhale Exercise Physiology is responsible for collecting personal information, which is stored electronically on a locked and secured computer. Personal information will remain disclosed, except when required under law or where there is a serious and/or immediate threat to the safety, health or well-being of the client or the public.

CONSENT:
I understand I am able to withdraw consent at any time (except when legal obligations must be met by Exhale Exercise Physiology).

CANCELLATION POLICY:
When scheduling an appointment with Exhale we value your time and ours. This cancellation policy ensures fair treatment for both you and our business.

Reminders: you will receive an SMS reminder 48 hours before your appointment.

Cancellations: We require at least 24 hours notice of cancellation for all appointments. This allows us to offer the appointment slot to another client. You will incur a 50% fee if you cancel within 24 hours of your appointment.

No-Shows: If you miss your appointment entirely without cancellation, you will be considered a no-show and will be charged a full fee.

Cancellation fee payment: We will send you an invoice to pay this fee or it will be added to your next appointment fee.

Exceptions: We understand that emergencies arise. We will waive cancellation fees for emergencies at our discretion.

By booking an appointment with Exhale, you acknowledge that you have read and agree to this cancellation policy.

EXERCISE TREATMENT:
Participating in Exhale Exercise Physiology may reap benefits including, but not limited to: improving cardiovascular health, decreasing overall fat mass, increasing muscular strength and hypertrophy, increasing quality of life through improvements in daily activities.
There are risks associated with all health procedure, assessments and exercise interventions. Such risks include, but are not limited to: fatigue, breathlessness, chest pain, muscle and joint strain and soreness, nausea, bruising and dizziness.
I understand that exercise is only prescribed with the best interest and safety in mind for me, the client. Exhale Exercise Physiology will only prescribe exercises within my capabilities.
If you have any concerns regarding prescribed treatment, please discuss these with Exhale Exercise Physiology.
